LINES OF AUTHORITY: This position is supervised by the Client Records
Supervisor. Unless specifically
designated, this position does not supervise any other personnel.
POSITION SUMMARY: The main function of this position is to manage the
clients Electronic Medical Record (EMR), including: Scanning records received from internal and
external sources; Inventorying physical charts
for long term storage and retrieve records in a timely manner; Processing the release of information
requests in a timely manner while adhering all laws and regulations including
HIPAA. Maintains client confidentiality.
PRINCIPAL DUTIES/RESPONSIBILITIES:
- Verifying
that scanning of client information is in proper order according to the current
filing format.
- Scanning
records, correspondence and miscellaneous forms received from an external
source in clients EMR.
- Sorting
incoming faxes.
- Completing
after-hours crisis reports according to specified procedure, including forwarding
messages to the appropriate staff member
- Requesting
and pulling clients' physical records from storage to ensure all information is
scanned and available to staff.
- Routing and
managing of client records according to policy
- Performs the
function of Release of Information as needed.
- The employee
must work well under pressure, meeting multiple and sometimes conflicting
deadlines. The employee shall, at all times, demonstrate cooperative behavior
with colleagues and supervisors.
- Works a
flexible schedule as needed to provide coverage at all sites.
- Participates
in the Performance Improvement Processes in the Clinic.
- Able to effectively de-escalate clients using techniques as trained and
outlined in approved crisis intervention models.
- Other duties
as assigned.

POSITION REQUISITES:
Qualifications and Experience
Required:
- High School
Diploma or General Equivalency Diploma ("GED")
- Must have a valid
Arizona driver's license
- Computer
experience, office machines, copy machine and facsimile machine
- Experience with
medical records or closely related tasks and requirements
Preferred:
- One year
experience working in the medical field
- Knowledge of Electronic Medical Records (EMR)
- Microsoft Office
software experience
- Knowledge of
confidentiality laws, HIPAA

Physical Requirements:

Equipment Operation:
Computer, scanner, laser
printer, facsimile machine, copy machine, shredder and telephone and the
ability to drive a vehicle to off-site clinics.
Environmental Conditions:
Air conditioned and/or heated
office setting
